Reporting state agencies for fraud to the State Auditor

Use the online form to report "Family and Protective Services, Dept of" (as form shows).  There are also other agencies that can be reported for fraud and abuse (check Texas Codes for more info)

Be sure to get their malpractice insurance information; policy provider, number & claims information when you start services.  If things get out of hand send the insurance provider a letter of intent to sue for damages.
